How health insurance affects your take-home pay
Employer-sponsored health insurance premiums are usually deducted pre-tax under a Section 125 cafeteria plan. This means your premium comes out before federal income tax, Social Security (6.2%) and Medicare (1.45%) are calculated. The result: every dollar of premium actually costs you only about $0.65 to $0.80 depending on your tax bracket.
Pre-tax vs. after-tax premiums
If your employer offers a Section 125 plan (most do), premiums are pre-tax — reducing both income tax and FICA. If you buy insurance on the marketplace or pay out of pocket, premiums are after-tax and provide no FICA savings. You may deduct marketplace premiums if you itemize, but you still owe FICA on the full gross.

Check your pay stub for the health insurance deduction line. It may be labeled medical, dental or vision. Your HR department or benefits summary should show the total premium and the split between employer and employee portions. Only the employee share reduces your paycheck.
Is marketplace insurance pre-tax?
No. Health insurance purchased through the ACA marketplace (Healthcare.gov) is paid with after-tax dollars. You may qualify for a premium tax credit that reduces the cost, but the premiums themselves do not reduce your payroll taxes.
